HAZOPtimizer IS A SOFTWARE PACKAGE FOR RECORDING AND MANAGING PROCESS HAZARD ANALYSES

Getting Started
To install the Excel macros template and Add-in:
-
Click on the supplied executable file HAZOPtimizer 6.0.exe
-
Follow instructions prompted by the dialog wizards
HAZOPtimizer installation puts the Excel template and Add-in into your folder at
C:\Program Files\ioMosaic\HAZOPtimizer 5.0.
To enable macro
For Microsoft Office 2003 user
-
Open Excel / Go to Tools / Macro
-
Select Security
-
Under the security Level tab, choose Medium for the security level and click OK
For Microsoft office 2007 user

If you click Options on the Message Bar, a security dialog box opens,
giving you the option to enable the macro.
To start using HAZOPtimizer:
-
Go to Start / Program Files / HAZOPtimizer
-
Select HAZOPtimizer 5.0 (click on "Enable the macros" if asked)

Select the HAZOPtimizer User Guide for additional help and information
regarding ioMosaic's products and services.
As you start HAZOPtimizer,the macros configure the template and insert any
required Add-ins automatically. To unload the Add-in, go to Tools > Add-ins
and unchecked the HAZOPtimizer option.

Understanding HAZOPtimizer Menu
When HAZOPtimizer is loaded, a new menu, HAZOPtimizer 5.0 will appear on the top
menu bar (Figure 1). There are four commands on the new menu, Show Toolbars,
Remove Toolbars, Help and About.

Show Toolbars opens three toolbars that are useful when recording PHAs.

Remove Toolbars removes the four toolbars created by the Show Toolbars command.

Help display help menu.
-
About displays a dialog box with HAZOPtimizer© 5.0 version and contact
information.

Understanding HAZOPtimizer Toolbars
HAZOPtimizer Toolbars may be turned on and off as needed. To enable
the Toolbars, click on the Show Toolbars under the HAZOPtimizer menu. Three
toolbars will open: Workbook
Functions,
Worksheet Functions, and
Utilities The toolbars
can be moved by dragging them to any convenient location on the screen (Figure
2). You are ready to go.

HAZOPtimizer Toolbar
To display the HAZOPtimizer 5.0 toolbars, click on
the command Show Toolbars (on the HAZOPtimizer menu). Three toolbars will open:
Workbook Functions,,
Worksheet Functions, and
Utilities. The toolbars
can be moved by dragging them to any convenient location on the screen. One bar
is for performing sheet operations such as inserting new sheets, renumbering
sheets, etc. The second bar has editing features such as add a line or remove a
line. The third bar has utilities for units conversion, calculating vessel
volume and for inserting preprogrammed common phrases, or comments. Below is a
description of each of the items on each toolbar.
Workbook Function Toolbar
| New Section |
This command will add a new section. The new section will appear in
the workbook after the last active study section. This new section is created
on a tab named Section X if it is HAZOP study or LOPA X
if it is LOPA study where X is the next number in the
sequence. If there are numbers missing from the sequence of sections, the new
section will fill-in the lowest missing number by default see Options Tab.
For HAZOP study section, all existing headings from Section 1 will be copied to
the new Section. The column headings can be changed to any description you
wish. |
| Copy Section |
This command copies the selected study section and inserts the
new study section in the location based on specifications in Options
Tab. |
| Remove Section |
This command deletes the selected Study Section or Study Sections.
Multiple sections can be deleted by highlighting all tabs to be deleted at the
bottom of the window.
Note: Do not use excel right-click on worksheet to remove
section.

| Switch Study |
This command will bring up the studies list window and allow you to
select other study type. After the new study type is selected, click on New
Section command.

| Renumber |
This command will renumber and reformat the table. |
| Lock |
This command allows information in a Study Section or a
Recommendation Summary sheet to be protected by a user password. The heading
and information in columns A to J are locked and inaccessible to change by an
unauthorized person. The columns Action and Responsibility on the
Recommendation Summary sheets are not locked and accessible to be used or
changed as needed during follow-up.
These macros use Excel protection and, therefore, the protection is limited by
any limitations that Excel has. The protection may only work when Excel is used
to access the workbook. The workbook may not be protected if some other
software is used. |
| Unlock |
This command allows access to previously protected Study Sections
or Recommendation Summary sheets. To unlock these areas requires the use of the
same password used to protect them with the Lock command above.

| Hide Header |
This command can be used to hide the header for the study section
being worked on. This provides more screen space for recording the study. |
| Show Header |
This command will show the header information for the study section
if it has been hidden. |
| Summary |
This command prepares summaries of the Study Sections. See the
"Create Summary" section (above) for a more detailed description of what this
command does.

| Prep for Print |
This command prepares the highlighted Study Sections and/or
highlighted summary worksheets for printing. It sets titles, page size, page
number, etc.

Worksheet Function Toolbar
| Add Line |
Add a line above the current line and automatically renumber the
table. The Add Line command only works in the table, below the column heading
(row eight).
Adding a line to the heading (using the Insert> Rows) may cause problems
with the macros. Don't add any lines to the heading rows (1 to 7). If more
space is required in rows 1 to 7, make the cells taller by using the usual
Excel click-and-drag sequence to increase the height of the row.
For HAZOP study section,all existing headings from Section 1 will be copied to
the new Section. The column headings can be changed to any description you
wish. |
| Remove Line(s) |
Remove an indicated line from the table and renumber the table. Be
careful, this can not be undone. The Excel command Edit> Delete. works, but
will not automatically renumber the table.

| Copy, Paste |
Highlight the cell or cells to be copied and click |Copy|.
Highlight the cell that is the upper left corner of the destination. Click
|Paste|. The copy and paste commands will work between cells in the same
worksheet, between worksheets in the same workbook and between different
workbooks. The regular copy/paste sequence in the Excel toolbar will also work
in the usual way.

Temperature, Pressure, Volume of Flow and Mass Flow Rate

| Volume Calc |
The Volume Calculator allows the entry of a spherical, horizontal or vertical
tanks dimensions and heads and the level in the tank. The calculator will find
the total volume of the tank and the volume of liquid at the level specified.
This volume can be in English or Metric units based on the input and requested
output. Just enter the tank dimensions and the units and click on compute.

Manual Configuration Option
You can also configure the HAZOPtimizer as part of your Excel
templates. This allows you start HAZOPtimizer from your Excel
application.
To add HAZOPtimizer as part of your Excel template:

Copy HAZOPtimizer5.0.xlt into your Office template folder. The
default location for this folder is: C:\DOCUMENTS AND SETTINGS\your user
name\APPLICATION DATA\MICROSOFT\TEMPLATES.

Open Excel
-
Under Templates, choose "On my computer..."
-
Choose HAZOPtimizer 5.0 as illustrated in Figure 3 (click on "Enable the
macros" if asked)

Using HAZOPtimizer 5.0
When HAZOPtimizer is loaded, an application wizard window will open, you can
choose to start a new study or open an existing study. When start a new study
is selected, studies list window will open to ask what type of study to choose
as shown in Figure 3

The following worksheets are shown as separate tabs after study type is
selected:

TOC: This is the table of contents of study sections. This
worksheet is updated when the Summary command is selected on the HAZOPtimizer
toolbar.

Attendance: This worksheet allows team members to be
listed and identify the study sections where they participated.

Section 1: This is the default study section which can be
HAZOP, FMEA, Checklist, Human Factors Checklist, and Facility Sitting
Checklist, What-If, Study Section Issues, Specific Equipment Issues and
Facility/Process Issues study base on the selection from application wizard.
The study type can be distinguished by its tab colors& (The color code is
listed in the Options tab).

LOPA 1:
This is the default LOPA study template.
-
Risk Ranking: This worksheet contains the default risk
ranking matrix, the team action table based on risk level, and the frequency
and consequence ranges for the risk matrix.

Deviation Matrix: This worksheet contains the deviation
matrix for each unique combination of HAZOP guide words and process parameters.
-
Comments: This worksheet contains the text of common
phrases that can easily be inserted into a cell by clicking on the Comments
command in the ioMosaic Utilities toolbar and selecting the appropriate phrase.
Additional phrases can be added here (up to 20 totals).

Options: This worksheet allows various features to be
selected to customize each PHA study.

Reference: This is for the LOPA study table references
Options Tab

Starting Section Number: this option allows the number of
the first section of a study to be selected (default is section 1).
-
New Study Section Number: This option allows a new study
section that is created to either be entered sequentially after the highest
study section number or to be created with the first available unused number.
-
New Study Section Guidewords: This second option allows
the user to select whether new sections that are created will contain the
contents of the default guidewords or the guidewords in section 1 (or the
lowest numbered section).

Print Preparation
: this option allows the user to select whether the print formatting will
include all worksheets or only the selected worksheets.
-
Comments/Questions column in summary: this option allows
the user to select whether the Comments/Questions column is shown on the final
worksheets.

Starting LOPA Number: this option allows the number of the
first section of a LOPA study to be selected (default is LOPA 1).
-
New Study LOPA number: This option allows a new LOPA study
section that is created to either be entered sequentially after the highest
LOPA study section number or to be created with the first available unused
number.
Entering Information
Click on the cell where the information is to be entered and start typing. The
Excel Edit> Copy (Ctrl+C) and Edit>
Paste (Ctrl+V) commands work normally. If the normal Excel
insert command is used to enter a new line, the Section will not be
automatically renumbered. Clicking on the HAZOPtimizer |Renumber| command
will renumber the Section for you.
Create Summary
At any time during data entry a summary can be created or updated. The summary
shows the HAZOP recommendations sorted by priority level and item number. The
|Summary| command collects all the recommendations of each
priority into a separate table for each priority level. Within each table,
items are sorted by item number.
The "Action" and "Responsible" columns do not appear in the Section tables.
Those columns are added to the summary tables.
To create the summary, simply click on |Summary| in the
HAZOPtimizer toolbar. The first time this is done, five worksheets will be
added to the workbook - one for each level of risk, High Priority, Medium
Priority, Low Priority, Very Low Priority, and one for Operability Issues. All
the recommendations will be sorted and copied from the study sections to the
appropriate sheet based on its level of risk. The last worksheet (Export to
ioXpress) compiles all recommendations into a single table to allow this data
to be exported to ioXpress for action tracking. All study sections are included
in to the summary sheets.
Operability recommendations are indicated on a Study Section sheet as 0 (zero)
Likelihood or Frequency and 0 Consequence. For this combination, the letter O
will be inserted in the risk column. This recommendation will be shown on the
Operability Recommendations summary described above and will be copied to the
summary sheet for Operability Recommendations.
In the recommendation column in which the phrase "No Recommendation" is included
the column will not be counted or copied to a risk summary table.
Printing
The normal Excel formatting and printing functions may be used, but a special
command has been included to save time in preparing the HAZOP sheets for
printing. Click on |Prep for Print|. The sheets will be
organized for printing in an easily readable format. To print the sheets, use
the standard Excel print commands. Check the options tab to select whether to
print the entire workbook or selected sheets.

Using LOPA
LOPA is a semi-quantitative risk analysis technique. It lies in between a HAZOP
and a quantitative risk assessment (QRA) in terms of its rigorousness. This
technique evaluates risks by orders of magnitude of the selected accident
scenarios and builds on the information developed in qualitative hazard
evaluation, e.g., PHA. The LOPA worksheets (Figure 6) allow user to reference
the scenarios identified under the PHA study sections.

PHA Reference
To link PHA item, click on the Get Item No button in the PHA Reference
cell, a dialog box will pop-up as shown below:
Consequence Type
To reference the PHA scenarios, enter the item number from the study section
worksheet and click OK.

Customizing HAZOPtimizer
Risk Ranking Matrix
The Risk Ranking Matrix in HAZOPtimizer© can be customized to match the users
preferred risk matrix up to a 5 by 5 matrix. The default matrix is 4 by 5. The
consequence and likelihood ranges must be represented by numbers 1 to 5. The
descriptions of consequence and frequency can be changed by typing in the
desired descriptive text for each.
Any symbol can be used for the risk level. A, 1 or I are all acceptable. The
highest risk is at the top of the table, the lowest risk at the bottom. An
operability issue is always represented as 00 (zero zero as characters, not
numbers).

Inserting a Digital Picture or Sketch
At times during a PHA it is found that some information of the P&ID's is not
correct, that information must be confirmed, or a sketch is needed to help
clarify a recommendation. In these and any other cases adding a picture or
sketch to the log sheet can be beneficial. In HAZOPtimizer© a digital picture
or sketch can be pasted to any cell by using the usual Copy and Paste commands.

Follow-up of Recommendations
Columns for "Action" and "Responsible" are
added to the Summaries of each risk level. This allows the recording of the
action and status of the recommendation to be recorded and also who the
responsible person, team or department is for completing action on the
recommendation.
Using the Excel Sort function lists of recommendation assigned for follow-up to
an individual person, team or department can be obtained. Also a list of
recommendation on which no action is to be taken can be obtained.
